Modification Application Form

Pool Pass Application

This year, we will be using a new pool pass system. All members must complete a new application to obtain their new passes. Members will receive ePasses at no charge, with the option to purchase printed passes.

Renters who wish to use the Avenel Swim and Tennis facilities will be required to provide an executed copy of their lease, including an Assignment of Use Form that assigns the right to use the facilities to the tenant from the owner/landlord.

Mailbox Insert
